There are definitely situations where it could make the difference. Doesn't chnage my personal feeling that taking a second to flip a safety or chamber a round may be the thing that keeps me from making a mistake that can cost me or someone else their life.

Many carry guns without any safety other than the trigger. I do. It is a revolver. The only safety is the long heavy trigger pull and it being in a holster that protects the trigger. It also never leaves my side unless it is to be locked up in the safe. Her problem was letting the pistol out of her control for even a second. It is sad that it had to cost her her life.

The gun she had was a S&W Shield 9mm. It has a safety similar to a glock, so in anotherwords not a true safety. People who carry often times don't want to have to mess with a safety or chambering a round. Her real mistake was keeping it in a purse. If you are going to carry you need to carry on your person. Doesn't do you much good if you leave your purse in the car or at home or God forbid your kid gets in there and put a round in your arse. It happened 18 miles from my house btw.

Not entirely correct, while the Shield does have a trigger safety similar to a Glock, it also has a side mounted thumb safety. There are now a few Shield's available without the thumb safety, but have only been available for a short time and are hard to find.

The thing I had read about the Shield didn't mention the thumb safety, I wonder if that's the version without. Still, the article I just read that talked about it made it sound awkward and a bit difficult. If hers had it it obviously wasn't engaged.
