A quick and dirty answer is this.

Both hold the tail stable however:

Rate mode - if the helicopter is hovering side ways to the wind, the tail will weather vain around so that nose ends up pointed in to the wind. When out flying around and you make a turn the tail will follow with little or no rudder input.

Heading hold mode - If hovering sideways to the wind, the helicopter will hold it's heading. The tail will NOT weather vain with the wind.

When flying around the tail will always hold the same heading unless rudder input is given. It will not just follow the heli in a turn. You have to 'Fly" the tail as well.

Google PID control loops. Rate mode is P only,.. what we call heading hold is a PI loop,.. but most have thrown in the D component to help starts and stops.
