dont know worn, lose belt a little lose and ridin on it those are just my guesses

Looks to me like the lower part of the pulley was wearing on the frame till the lip wore right off it! The correct spacers were installed on the shaft, & I haven't figured this one out yet, but it's going to be part of my pre-flight checklist from now on.

like i said things just wear out. intereseing though that it happened multiple times and did not just destroy the belt like it did on mine dont know. that said i would check to make sure every tooth on the belt is still intact.

Ive figured out exactly what the problem was! I flew 2 tanks the other day with no problems. OK first what you wanna do is tighten the boom so you have the correct tail belt tension. I keep mine fairly snug. Next and this is the BIG key you wanna tighten the tail boom supports (this is much easier with 2 people) on the back wing while at the same time holding the heli landing gear flush on the table and pushing up on the back of the tail boom. What this does is give the tail boom a slight upward angle causing the belt to ride in the middle of the pulley. I even had the tail boom to high once and the belt was riding all the way on the top side of the pulley. The way you check it is to spin the mail gear clockwise and simply look at where the belt is on the left pulley. If the belt is all the way at the top of the pulley let the boom come down a touch and if its still at the bottom of the pulley push the boom up a bit more and tighten the supports.
What I was doing wrong was pushing down on the top of the back tail boom support screws to tighten them and I had to push hard because the screws were kinda stripped which was putting my tail boom at a downward angle. Well Im glad Ive finally figured this problem out. Its caused me 3 crashes!

Glad you found the problem!!

I've had 2 instances of the belt jumping down off the pulley on the LEFT. Not knowing it had happened, I carried on with a short flight, and noticed this incredible amount of dust all over the heli mechanics. Second time I was smarter, and stopped straight away.

Cause? All I could think of was a belt that wasn't quite tight enough (a cold day and a shrinking boom?), and on hard auto landing the belt flipped off the pulley. With tail and main rotors slowing, there'd be little tension in the belt on the left side.

I can't see how missing belt teeth could contribute to the forward pulleys wearing their edges on the side frame. Can you explain ?

reverse it wearing edges could cause missing teeth.
