How far up the mainshaft do you set the swashplate follower? Half the available travel between headblock bottom and the swashplate bottomed out on the bearing block, or up against the bottom of the headblock?

Doesn't matter.
If you setup the bird for 3D and have the same +- collective pitch range, then set the arms of the swash follower to be level at zero pitch.
