Must be my lucky day. I do a through post and pre flight.
Cleaning up the slime after my second flight today, the head "caught" while I was rotating it.

LOOKED AT THE JEASUS BOLT, THE NUT WAS GONE, THE BOLT 1/2 WAY OUT!

It was checked in pre flight.
Dodged a big one today.

Nice save! The bolt did not pop out during flight probably because the head was being pressed against the bolt while loaded. Would be a different thing on an auto...

That used to happen with my Century Falcons and Ravens.

Cut a longer bolt so that the shoulder clears the end of the head, adding washers as necessary.

I have always used custom cut bolts. Cut so the smooth shank passes all the way thru the assembly, no threads on the ID. I am wondering and have wondered if the nylock nuts wear out after X amount of installations. Up till now Ive reused em indefenantly. Its the only "cause" I can think of. This heli is very smooth, no vibes.

I hear ya Marty, had that bolt been thrown the blade issue would be the least of my problems! As it is I guess Ill order a stock of new nylock nuts, play the numbers tonight, thank my very lucky starts and mark this up as lesson learned. One of many, most were alot more expensive.

Oh and I wish I could mark this down as a save but just luck this time

nylock nuts only work 1 time after that they still "kinda" work but should be replaced

oh I see, it was the nut that fell off, not the bolt that severed.

My bad.

I wouldn't worry about the lottery. It sounds like you used up all your good luck on that one.

Some where on here is a video of a guy taking off when the head came off. It was impressive and funny as the head flew away for 100 s of feet.

EDIT: Here it is.

It happened to me with my Rappy 30 a couple of weeks back. One Sunday afternoon I got 5 flights in bacvk to back - generally hooning around, full bore across the flying field, coming in fast to a hot LZ - you know etc etc After I'd shut down, having landed from that 5th flight I wnet to pick up the Rappy by the rotor head and it moved up there was NO JESUS BOLT AT ALL...

Tach.
