In all fairness, I did enjoy an inverted pile driver into the hard dirt. The heli faired very well but.......
Several flights after my rebuild, the seesaw mixing arm came off of the bearings during flight and well we all know what happened then. Lucky that I was landing and was only a few feet off the ground.
I found that the small rib of metal that separates the two bearings and keeps the arm from sliding off the outside bearing had sheared off. After further inspection, one of the washout arms was sloppy on the bearings. It would move in and out on the bearings. I found that the washout arm had sheared off about half of the rib and it was just hanging in the washout arm. If I had not inspected this arm, it would have failed within a flight or two. Take a look at yours! Are they sloppy?
I take full reponsibility for not completely inspecting my heli after my crash but has anyone else experienced this type of failure?

Could be lack of loctite!
During inspection of our second Beam e4 we found that one of the washout mixing arms had the same problem. The rib was gone and would have failed in flight. This heli has no crashes and 10+- flights.
We did not loctite any of the bearings in the mixing arms. Guess we missed that step on both heli's. All are replaced and bearings are secure with loctite. We learn by our mistakes. If you did not use loctite on yours, you may want to.
I am not positive that this will cure the problem but it seems logical.

Yes, definitely check the bearings in the mixer and seesaws!! I was noticing that during a few flights, mine was a bit erratic, which was not normal.. I looked everything over and never saw a thing. I started pulling on things and noticed the bearing pull away from the arm. I checked 'em all and found that both sides were coming out. Luckily I added a dab of CA and it locked it in there solid, but just make sure you all check because it can lead to an in-flight disaster or injury for sure!

Thanks for the additional info oteskate.
I ordered bearings that are the same size, except not as deep (.6mm shorter because of a flange). I will have to use a little longer bolt and possibly install a shim or spacer between the inner and outer(flanged) bearing. I will post my results. If the bearings work, this can never happen again. Although, after putting loctite on the bearings, all four arms still seem rock solid after 10+ flights.
