If you examine the heads that have come apart during drag racing or from any other activity that has high head speed, the failure typically occurs in one of 3 places.
1 - The blade grip fails - typically plastic grips are a no no.
2- The threads pull out of the spindle
3 - The head of the cap screw pops off.
I have done several tensil tests of rotor head assemblies showing where and how they break and also the amount of force it takes to make it happen.

The bottom line for safety is -
- use light weight, high quality blades, "V" blades are a good choice.
- Change your spindle shaft and bolts often to reduce the potential of stress changing the characteristics of the material
- Change your blade grips to metal
- Change the blade bolts often
- Run a governor to limit the max head speed.
- Calculate the actual force your blades are pulling, I believe Nate? has a good online formula.
